DENIAL MANAGEMENT · CLAIM RECOVERY
Turn denied claims into collected revenue.
WHY CLAIMS GET DENIED
The top reasons revenue slips through.
Eligibility & coverage
Inactive coverage, wrong plan or missing benefits caught too late, after the visit has already happened.
Coding errors
Wrong, missing or mismatched CPT, ICD-10 and modifier codes that payers reject on sight.
Missing authorization
Prior authorizations and referrals that were never obtained, expired or did not match the service billed.
Timely filing
Claims and appeals that miss payer deadlines, turning recoverable money into a permanent write-off.
Incomplete documentation
Notes that do not support medical necessity, so the payer asks for records and the claim stalls.
Duplicates & COB issues
Duplicate submissions and coordination-of-benefits conflicts between primary and secondary payers.

FAQ
Denial management questions, answered.
Denial management is the process of tracking denied or rejected claims, finding the root cause, correcting and resubmitting or appealing them, and fixing the process so the same denial does not repeat.
AR recovery follows up on unpaid claims that are still pending or aging. Denial management handles claims the payer has already rejected, and focuses on why they were rejected and how to prevent it.
The usual causes are eligibility problems, coding errors, missing prior authorization, missed timely filing limits, incomplete documentation, and duplicate or coordination-of-benefits conflicts.
It depends on the payer and the plan. Many allow from 60 to 180 days from the denial date, which is why we work every denial as soon as it arrives.
Yes. Our free denial audit looks at your recent denials, groups them by payer and reason, and shows how much is recoverable and what is causing it.
